Method statement for external drainage works

Stage 1:Preliminaries and General Work

A. Contractor shall submit the proposed material and related miscellaneous item  as following item to    superintending officer (S.O) or his representative (S.O.R) for his approval.

1. 
   
2. Concrete mix design
3. Steel bar mill certificate
4. Detail Work Program
5. Reinforce concrete pile specification
6. piling set calculation
7. method statement for test pile and loading test
8. Quality control and Quality plan

Stage 2 : Construction Work Method statement

1. Prior to any excavation works and any other works related to external drainage system contractor shall prepare and proceed with the drain alignment and invert level setting out.
2. Upon completion of the setting out, Request for inspection form shall be submitted to superintending officer (S.O) or his Representative (S.O.R) to allow inspection being conducted before any subsequent work start. 
3. The excavation works can only start upon receipt of official acceptance for the setting out and approval to proceed works with subsequent activity.The Official reply on the inspection can be recorded in the submitted Request for inspection work or by any other means preferred by S.O/S.O.R.
4. Excavation work shall be done using combination of manual Labour and machinery.
5. Upon completion of the excavation work, contractor once again shall submit to S.O/S.O.R Request for Inspection form to enable inspection being conducted to confirm that the invert level of the drain are as per requirement.
6. Contractor then proceed with the setting out of the pile location for the piling work upon acceptance and approval to proceed with subsequent work from S.O/S.O.R.
7. When ever Contractor feel that the setting out for pile position have reach the numbers that is practical for them to proceed with pile driving work, Contractor shall once again submit the Request for Inspection  to enable inspection being carry out by S.O/S.O.R for the pile location setting out.
8. During the inspection and upon acceptance of the setting out by S.O/S.O.R, he shall determine the particular location for the test pile and pile loading test.
9. Contractor shall proceed with the first pile location that is the test pile location. There shall be no further pile driven work being carried out before the result of the test pile and pile loading test is approved and accepted by S.O/S.O.R . The Result MUST fulfil the design requirement.
10. Once the test result is deem to be in compliance with the requirement and officially approved and accept by S.O/S.O.R , Contractor then can continue with the remaining Pile drive in work and followed by cutting off the excess pile. 
11. All pile that are going to be used shall be properly inspect by S.O/S.O.R for any defect prior to piling works. Shall there be any unsuitable pile being detected, S.O/S.O.R shall give official instruction to contractor to remove out the pile from site as to prevent defective pile being use for the permanent works.
12. There shall be no piling works being carried out with out supervision of S.O/S.O.R.
13. Upon completion of the piling work, Contractor then proceed with the structure work of the drain.
14. the subsequent work are as follows;

• preparation of pile head
• laying of granular bedding
• laying of the lean concrete
• installation of the steel reinforcement for the drain
• installation of the formwork
• drain structure concreting work
• Curing works for the concrete
• removing the formwork
• Inspection to the concreted drain to detect any minor and major defect for immediate rectification work
• Inspection for the final invert level as to verify the constructed drain invert level are as per requirement.

15. Concreting work shall not being carried during raining day.

16. All inspection shall be conducted by S.O/S.O.R in the present of contractor.

17. Sample of concrete that being delivered to site shall be taken  for every 5 cu.m delivered batch and being                 tested for 3 day, 14 days and 28 days strength to make sure that the concrete use is as per requirement.

18. Shall the concrete grade is found to be of the lower grade, S.O/S.O.R shall request the contractor to do the rectification work according to a proper method requested by S.O/S.O.R.

WORK METHOD STATEMENT FOR LIFT HOLE/TRENCH/PIT EXCAVATION WORK

1. Prior to the excavation work setting out for the Lift hole/Pit shall be established. The setting out shall include the plan dimension and invert level as shown on the approved construction drawing.
2. Upon completion of the setting out, Approval Chit/Request for inspection shall be submitted to the superintending Officer (S.O) or his/her representative (S.O.R) to enable them to do the inspection and verification on the setting out. No excavation work shall commence without any official instruction from S.O/S.O.R.
3. Excavation work shall only commence upon acceptance of the setting out was done according to the construction drawings by the S.O/S.O.R.
4. To avoid any unwanted incidents, Safety sign such as “Awas” and Safety tape shall be install where necessary.
5. There shall be no excavation work been carried out during raining days.
6. Anytime, during the excavation of the Lift trench/hole/pit, any machine/plant or equipment shall not be place at the edge of the trench/hole/pit. This is to prevent the vertical earth-wall from collapse and cause injury to the worker.
7. As soon as possible, after the completion of excavation work, timber prop shall be erected to prevent the collapse of the vertical earth-wall before permanent concrete wall is constructed.
8. All excavated earth shall be transported out to contractor dump site or Stock pile at the approved location within the site for later use. 

Proposed Machine/plant/equipment: 

1. Excavator/Backhoe
2. Dump truck/tipper truck
3. Water pump.

Method statement for site clearing

A). Work Method Statement

1. Prior to the commencement of the site clearing Contractor shall establish the site boundary. The setting out of the site boundary shall be done by an approved license land surveyor.
2. Upon completion of the site boundary survey and pegging works, a copy of the boundary setting out survey drawing shall be submitted to superintending officer for the project for his verification and approval. 
3. Contractor shall also submit their proposed dumping site for approval prior to site clearing to the superintending officer.
4. There shall be no clearing works started before any approval being issued out officially by the superintending officer.
5. During the survey and setting out of the site boundary, contractor will inspect if there is any existing existing crops or property that is within the site. If there is any, contractor shall notify the superintending officer officially and request for information whether the land accusation process, crops counting process, and identification of the affected property had been finalised and compensate accordingly to the owner of such item.
6. In the event that only land accusation had been conducted and finalised for payment by the authority, Contractor shall request to Superintending Officer to perform joint inspection and measurement between the parties involve which is the owner of the crops and property, Superintending officer or his representative, relevant authority and also the contractor. All parties MUST present during this process to witness and agreed on the data collected. All of the recorded data shall be signed by all parties.
7. Recorded data shall contained the information on the location, Ownership, quantity, description, date, photo (if possible) and shall be signed by all parties.
8. Contractor, after the joint inspection then shall notify superintending officer officially and request approval to proceed with the site clearing works. 
9. All clearing activities shall be recorded in site diary on daily basis.
10. Contractor shall erect sufficient safety sign to notify public on the construction activities shall any need arise.
11. Contractor MUST abide to the environmental act or any statutory and regulatory requirement through out the clearing activities.
